During the ordering process you can upload any release forms that are required. If you don’t have a signed release form ready to upload you can complete your order and come back latter to add it.

To upload a release form:

1. Go to the reports menu and choose Pending Reports.
   
2. Select the report you need to add a release form for.
   
3. You can then add the release form as an attachment to the report.
   
4. Release forms are required to be uploaded for PSP Reports and MVR Reports on licenses issued by New Hampshire, Ohio, Pennsylvania and Washington. These searches will not be ran without first uploading the required release form.
